Patient Mapping Scenarios While Attaching a Memo
| Scenario | What it means | What you need to do |
| Single Patient Linked | Phone number is linked to one individual patient. | The memo is auto-mapped. |
| RP (Responsible Party) Linked | Phone number is linked to a Responsible Party (RP) who manages appointments for multiple patients. | Since the RP’s number is linked to multiple patient profiles, the system cannot automatically determine where the memo should be attached. Therefore, you must manually select the correct patient profile to attach the memo. |

Transcript Generation
- Once the Call Transcript is generated and the IQ Summary is created.
- This replaces the need for separate email updates or manual notes.
How to Attach a Memo to a Patient
Case 1: For an existing patient
- The IQ summary of a call is automatically attached under the Memo section of the patient profile in the PMS.
Case 2: For a patient number that is not linked with any patient in CareStack PMS
1. From Patient 360, click Attach Memo.
2. Use Search for Patient to select the appropriate patient (or RP/multiple patients).
| Note: If this is a new patient, the patient would be listed under the search after you create a patient profile in the CareStack PMS. |
Case 3: If a patient number is linked to multiple patients in CareStack PMS
- Once you click Attach Memo, a dialog box titled Add IQ Summary to memo appears.
- Select the patient profiles you want to link to the specific patient’s number, and then click Send to memo.
After Saving:
The memo is synced to the selected patient profile in the PMS.
The Attach Memo button becomes inactive to prevent duplicate entries.
Memo Handling for RP-Linked Calls
For Responsible Party (RP)–linked cases, agents must manually attach the memo.
If the memo is not attached, the call will appear in the Memo Update Pending option inside the Call History side panel, as shown below:
